Children's Community Paediatric Occupational Therapy And Physiotherapy Service

A specialist managed and co-ordinated Community Occupational Therapy and Physiotherapy service for children who have:

− Physical

− Sensory

− Perceptual and/or cognitive difficulties

and an identified therapy need following assessment by the service.

The service works with young people who are cognitively able up to age 25 if required.

Children with learning disabilities who continue to need support in adulthood transfer to Adult Learning Disability services at age 18.

Eligibility information


Children and young people with identified needs living in Amber Valley, Erewash, South Derbyshire and South Derbyshire Dales and who are registered with a Derbyshire GP in those areas.

Referral route:

Referrals are accepted from GP's; Hospital or Community Paediatricians; other Children's Therapy Services and Social Care Teams; Specialist Nurses and Health Visitors; SENCo's

Transition arrangements:

Children transition into our Young Adult Team (YAT) at 14 yers (if they are cognitively able) and transfer to adult servies at 25 years if required.

Children with learning difficulties transition to Adult Learning Disability services when they are 18 yrs.
