The service is delivered through families, carers and/or other key workers who require specialist support or advice to support a child to participate as fully as possible in everyday life. Address details Chesterfield Royal Hospital NHS Foundation Trust The Den Calow Derbyshire S44 5BL Telephone 01246 514509 Email address email@example.com Website address www.chesterfieldroyal.nhs.uk Age groups Birth To 18 years This provider is open during:All Year Additional cost or availability information The service can take place in a variety of community settings across North Derbyshire including: - The Child Development Centre (The Den) at Chesterfield Royal Hospital - Local Health Centres - Mainstream schools and nurseries - Special school settings - Families homes Service offered to children and young people up to aged 18, or aged 19 if in full-time education Eligibility information Eligibility: Children (aged 0-18 yrs) who are registered with a North Derbyshire GP, and are normally living in North Derbyshire. Referral: The Service has an open referral system, which means that any key person in the child's life is able to refer, as long as the child's legal guardian has given permission. This is why there must be a parent/carer signature on the referral form. Referral form and checklist completed by health or education staff must be discussed with parents and parental signature giving consent. Referral form can be completed by parents/those with parental responsibility. Transition arrangements: − Following discussion with parents/carers the SLT will refer children to other support services as appropriate. − Speech and language therapy will contribute to the Transition planning coordinated by the child's school.
