The service is delivered through families, carers and other key workers who require specialist support to help their child to participate as fully as possible in everyday life. Address details Chesterfield Royal Hospital NHS Foundation Trust The Den Calow Derbyshire S44 5BL Telephone 01246 514509 Website address www.chesterfieldroyal.nhs.uk Age groups Birth To 18 years This provider is open during:All Year Additional cost or availability information Service is held at a variety of community settings across North Derbyshire, including: - The Child Development Centre (The Den) at Chesterfield Royal Hospital - Mainstream schools and nurseries - Special school settings - Families homes Eligibility information Eligibility: Children (aged 0-18 yrs) who are registered with a North Derbyshire GP, and are normally living in North Derbyshire. Referral Route: The Children's Physiotherapy Therapy Service accepts written referrals from GP's and other health professionals as long as the child's legal guardian has given permission/consent. Transition Arrangements: - Following discussion with parents/carers the Physiotherapist will refer children to other support services as appropriate. - The Physiotherapist will contribute to the Transition planning coordinated by the child's school. - The GP, parents and all key workers are informed in writing when support from Physiotherapy service has ended. - Sign post children/families to Adult services where these exist. We are part of an integrated service and have close links with our colleagues in CAMHS, Paediatrics, Occupational Therapy, Speech and Language Therapy and School Health.
