Continuing Healthcare

Continuing Care is free care outside of hospital that is arranged and funded by the NHS.

The care can be provided in any setting, including residential schools, residential placements, hospice or the home of the person you look after.

The service complete assessments against the adult criteria for continuing healthcare for young people with severe or complex needs.

On-going case management and review for eligible young people.

Eligibility information


Young people in transition. For the eligibility criteria used for the assessment please contact the service.

Referral route:

Checklist and Nursing Assessment to be forwarded to single point of access. Fax: 01246 514159 or email.

Transition arrangements:

If the young person following assessment is eligible we work as part of the MDT with teh young person and their family to identify services and opportunities to meet their health needs.

We carry out a review at three months and then every year unless things change sooner.

If services are not meeting needs we can look at alternatives and plan for the next transition eg. leaving school.
